Owner / Therapist Identity Proof
Required for all registrations, bank accounts & license applications
  • Aadhaar Card (mandatory for GST and most state licenses)
  • PAN Card — serves as both personal and business PAN for a sole proprietorship
  • Passport-size photographs (6–8 copies recommended)
  • Address proof — recent utility bill, bank passbook, or rental agreement in your name
🎓
Cupping / Hijama Certification
Required by municipal bodies for Health Trade License issuance

Municipal corporations increasingly ask for proof of qualification before issuing a Health Trade License. Your certification from a recognized institute (like CupCure's own diploma program) serves this purpose.

  • Diploma / Certificate in Cupping Therapy or Hijama Therapy from an accredited institute
  • 12th standard (Higher Secondary) equivalent to the 10+2
  • IPHM, NSDC, or equivalent international accreditation (optional but valued)
ℹ️
Note for Maharashtra: The Maharashtra Acupuncture Council permits registered acupuncturists to practice cupping therapy. This may serve as an alternate pathway.
🏢
Sole Proprietorship Setup
No mandatory central registration — but operational proofs are required

No formal incorporation required. You need at least two of the following to prove business existence:

  • PAN Card of proprietor (serves as the business PAN too)
  • GST Registration Certificate (if applicable)
  • Shops & Establishments License
  • Udyam (MSME) Registration Certificate
  • Chosen trade name (e.g. "ABC Wellness") — check trademark availability
🏪
State Shops & Establishments Act License
Must be obtained within 30 days of starting operations

Apply to your local municipal office or state labour department. Required before you can get the Health Trade License.

  • Filled application form (from municipal authority / state labour dept.)
  • Identity proof of owner (Aadhaar + PAN)
  • Address proof of the premises (rental agreement / ownership deed)
  • Nature of business description
  • Working hours and number of employees (self, in this case)
🏥
Municipal Corporation Health Trade License
Certifies hygiene, sanitation & fitness for public service

Issued by your local municipal body (e.g., BMC in Mumbai, BBMP in Bangalore). Physical inspection conducted before issuance. Renewal required annually.

  • Filled application form from municipal corporation
  • Floor plan / layout plan of the treatment center
  • List of services offered (dry cupping, wet cupping, etc.)
  • Your qualification / certification certificate
  • Identity proof of owner + premises rental agreement or ownership deed
  • Shops & Establishments Certificate (prerequisite)
  • NOC from landlord (if rented premises)
🔑
Location & Space Compliance
Premises must be approved for commercial use
  • Rent / Lease Agreement (registered, if possible)
  • Landlord's NOC (No Objection Certificate)
  • Proof of commercial use approval for the property
  • Electricity bill / utility bill in premises name
  • Property tax receipt (if owner-occupied)
⚠️
The premises must not be in a purely residential zone. Operating from a residential flat without commercial approval can lead to license rejection or cancellation.
🧯
Fire Department No Objection Certificate

Apply to your local fire brigade. For a small solo center in a commercial building, the building's existing fire NOC may satisfy the Health Trade License inspector — attach a copy of it with your application. A unit-level NOC is typically needed only if you use open-flame cupping or are on an upper floor.

  • Filled NOC application form
  • Floor plan of the premises
  • Proof of fire extinguisher installation
  • Emergency exit plan
💡
Already in a commercial building? The building's existing fire NOC often satisfies inspectors for small ground/first floor units. Keep a copy handy.
📊
Goods & Services Tax Registration
Recommended even if below threshold — enables professional invoicing
  • PAN Card
  • Aadhaar Card
  • Address proof of business premises
  • Bank account details (cancelled cheque or passbook)
  • Passport-size photograph
🩸
Biomedical Waste Disposal Agreement
Legally required when wet cupping involves blood extraction

If you perform wet cupping (Hijama bil-shart) involving skin incisions and blood extraction, used blades, cups, and cotton are classified as biomedical waste under the Biomedical Waste Management Rules, 2016. Improper disposal is a criminal offense.

  • Tie-up agreement with a CPCB-authorised biomedical waste disposal agency in your city
  • Yellow, red, and white color-coded waste bins on-site as per BMW Rules 2016
  • Sharps disposal container (for lancets/blades) — puncture-proof, red-lid
  • Waste disposal log / register to be maintained at the center
  • All single-use items (cups, blades, gloves) discarded after every session
⚠️
Never reuse blades or lancets. Single-use sterile equipment must be documented. Disposal records may be inspected by health authorities.
📋
Client Documentation
Required by law for all wellness centers in India

Wellness centers are legally required to maintain a register of all clients. Especially enforced in cities like Mumbai, Delhi, and Bangalore.

  • Client intake form: name, address, contact number, age, gender
  • Informed consent form — covering treatment nature, risks, contraindications, and client agreement
  • Medical history questionnaire (blood disorders, pregnancy, medications)
  • Session log / treatment record per client
